The Chronic Disease Management Programme aims to improve the health and well-being of patients living with specific chronic diseases, including Type 2 diabetes, Asthma, COPD and Cardiovascular disease.  This service is provided by Avenue GP to ensure patients’ conditions are actively monitored to improve management of their conditions.

You are eligible to join this programme free of charge if you:

• Have a medical card or a GP visit card
• Have a specific chronic disease(s):
  • Type 2 Diabetes
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Cardiovascular Disease including: – Heart Failure – Heart Attack (Angina) – Stroke – Irregular Heartbeat (Atrial Fibrillation)

As part of this programme you will be invited to attend two appointments with your doctor and two appointments with your nurse per year, where blood tests an other investigations specific to your condition will be performed and a management plan will be agreed.
